Private Health Services Plans are available to all Canadian corporations, partnerships, and sole proprietors. If you are located outside of Canada, a PHSP is not available to you.

Main Company Contact Details

Enter details for the person that HealthRabbit can contact for information on your plan. If you have multiple people, or someone fills in when you're on holidays, you can shoot us an email and we'll add it to your file.

PHSP Details

Enter the last fiscal year end date for the business. A PHSP cannot process any claims before the last fiscal year of signing up and therefore, your effective date cannot be be before your last fiscal year end.


Enter the date you would like to be considered your year-end date for employees to submit claims. If you are a sole proprietor, this date will be set to January 1.


A PHSP can allow claims to the beginnning of the fiscal year. Enter the earliest date you'd like to allow employees to make claims in this fiscal year.


Coverage Classes

For each class of employee you are providing coverage for, enter the maximum amount they may claim in a year. If you are a one-person business, you may leave this section blank and you will simply have "Class A- Unlimited".

If you require more classes, please contact us and we'll add them to your account.


There are two ways to pay for your PHSP:

Our recommended way, and the fastest for your employees is on a Funded basis. We will calculate the number of employees by the levels you have selected and set high and low thresholds for your business to maintain. This allows you to manage your cashflow and ensure that your employees claims are paid out within 1-2 business days. 

You may also choose a Scheduled plan. Claims are bundled and then invoiced to your business on a set schedule (monthly or quarterly). When the invoice is paid, claims are paid out to you and your employees. Recommended for people who are self-employed and only want to deal with their own claims more infrequently.

You will choose Funded or Scheduled on the next page. Scheduled plans may move to funded at no cost, anytime. Funded may move to a scheduled plan for a fee. 

Employee List

You can upload your employee information now using this file. We'll create accounts for your staff and send them an email to set their password so they can start making claims. You may choose to continue enrolment and complete the employee list later.
